New Patient Registration
Patients living within the practice area are welcome to register. This covers all GL10 postcodes and many GL2 and GL5 postcodes. If you are unsure you can speak to a receptionist or take a look at the map below.
You will need to complete a registration form (GMS1) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can print off the form from the link below so that you can complete it and bring it to the surgery at your convenience. Please bring your Medical Card if you have one and also proof of identification (photographic if possible) and a household bill if available.
Disabled Patient Facilities
Disabled patient facilities are also available at this practice, car space, ramp, toilet etc.).
Change of Personal Details
Once registered at the surgery, should you change your name, address or telephone number at any time, please let us know by contacting reception or writing to the surgery. It is also helpful to let us know a change of mobile number as soon as possible.
If you move outside the practice area you will be asked to find another doctor nearer to where you are living.
